[摘要] 目的 確定斜視兒童生存質(zhì)量量表（Child-IXTQ）在斜視兒童術(shù)前生存質(zhì)量評(píng)估中的適宜臨界值。 方法 對(duì)106例斜視兒童術(shù)前進(jìn)行Child-IXTQ和兒童生存質(zhì)量量表（PedsQL4.0）的測(cè)評(píng)，并以PedsQL4.0作為測(cè)評(píng)Child-IXTQ的效標(biāo)，通過對(duì)Child-IXTQ各分值對(duì)應(yīng)的靈敏度，特異度計(jì)算繪制受試者工作特征曲線（ROC曲線）得出Child-IXTQ在斜視兒童術(shù)前生存質(zhì)量狀況評(píng)估中的應(yīng)用，從而確定最佳臨界值。 結(jié)果 Child-IXTQ的得分與兒童生存質(zhì)量量表PedsQL4.0表得分呈正相關(guān)（r = 0.609，P < 0.01）；當(dāng)Child-IXTQ臨界值為67分時(shí)，受試者工作特征（ROC）曲線在左上方離標(biāo)準(zhǔn)參照線最遠(yuǎn)，此時(shí)靈敏度、特異度分別為0.929和0.893，ROC曲線下面積為0.902（P < 0.01），標(biāo)準(zhǔn)誤差為0.052。 結(jié)論 Child-IXTQ在斜視兒童術(shù)前生存質(zhì)量評(píng)估中的最佳臨界值是67分，是斜視兒童術(shù)前生存質(zhì)量測(cè)評(píng)的有效工具。

　　[Abstract] Objective To identify the optimal critical value of the child intermittent exotropia questionnaire （Child-IXTQ） in preoperative quality of life assessment for strabismus children. Methods A prospective cohort of 106 strabismus children were recruited to complete the Child-IXTQ and PedsQL4.0 questionnaire， and the PedsQL4.0 was used as the criterion for evaluating Child-IXTQ. Through the sensitivity and specificity calculation of each score corresponding to Child-IXTQ， we draw the receiver working characteristic curve （ROC curve） to get the application of Child-IXTQ in evaluating the quality of life of strabismus children before operation so as to determine the best critical value. Results The score of Child - IXTQ was positively correlated with the score of PedsQL4.0 （r = 0.609， P < 0.01）. The optimal critical value for Child-IXTQ was 67 scores， the subject"s operating characteristic （ROC） curves is the farthest from the standard reference line in the upper left. The sensitivity and specificity were 0.929 and 0.893 respectively， the areas under the curve was 0.902 （P < 0.01）， and the standard error was 0.052. Conclusion The optimal critical value of Child-IXTQ for the strabismus children is 67 scores， and it is an effective tool for preoperative assessment for quality of life of strabismus children.

　　斜視是兒童易患的眼部疾病之一。它是指一種視軸分離狀態(tài)，即兩眼注視同一目標(biāo)時(shí)，一眼注視目標(biāo)，另一眼偏離目標(biāo)[1]。據(jù)統(tǒng)計(jì)，我國兒童斜視發(fā)病率為3%～4%[2]，呈逐年上升趨勢(shì)。斜視不僅會(huì)引起患兒個(gè)人形象紊亂和立體視覺障礙，還會(huì)導(dǎo)致患兒心理上出現(xiàn)抑郁、孤獨(dú)、低自尊等性格缺陷。他們?cè)趯W(xué)校受到無情的譏諷和嘲笑，在選擇玩伴時(shí)遭到排斥，從而造成其生存質(zhì)量明顯下降[3-5]。斜視兒童生存質(zhì)量量表（Child-IXTQ）是由美國研究者Hatt等人開發(fā)，廣泛應(yīng)用于斜視兒童生存質(zhì)量的研究[6-7]。但Child-IXTQ尚未界定臨界值，這將不利于斜視兒童生存質(zhì)量現(xiàn)狀的了解和干預(yù)效果的評(píng)價(jià)。鑒于同一種疾病不同階段所得問卷的最佳臨界值各不相同的特點(diǎn)，本研究以術(shù)前時(shí)段為觀測(cè)點(diǎn)，旨在探索Child-IXTQ在我國斜視兒童術(shù)前生存質(zhì)量狀況評(píng)價(jià)的最佳臨界值。
