慢性腎衰竭尿毒癥應(yīng)用高通量血液透析與血液透析濾過(guò)進(jìn)行治療的效果比較

【摘要】 目的:探討血液透析濾過(guò)、高通量血液透析應(yīng)用于慢性腎衰竭尿毒癥患者治療臨床效果。方法:選取2016年3月-2017年4月本院收治的慢性腎衰竭尿毒癥患者82例。按照隨機(jī)數(shù)字表法將其分為觀察組(血液透析濾過(guò))、對(duì)照組(高通量血液透析),各41例。比較兩組臨床療效、并發(fā)癥、治療前后的血肌酐水平、免疫球蛋白水平及肺功能指標(biāo)。結(jié)果:觀察組治療總有效率為97.56%,高于對(duì)照組的82.93%( 字2=4.986,P=0.025),觀察組并發(fā)癥發(fā)生率14.63%,低于對(duì)照組的41.46%( 字2=7.312,P=0.007);治療后,兩組血肌酐、RV水平均低于治療前,IgM、IgA、IgG、FVC、VC、TLC水平均高于治療前,且觀察組血肌酐、RV水平低于對(duì)照組,IgM、IgA、IgG、FVC、VC、TLC水平均高于對(duì)照組,比較差異均有統(tǒng)計(jì)學(xué)意義(P<0.05)。結(jié)論:血液透析濾過(guò)、高通量血液透析應(yīng)用于慢性腎衰竭尿毒癥治療,均可使患者免疫功能得到有效提高,促進(jìn)患者肺功能得到有效改善,但與血液透析濾過(guò)相比,高通量血液透析治療效果更加顯著。

【Abstract】 Objective:To investigate the clinical effect of high throughput hemodialysis and hemodialysis filtration in chronic renal failure uremia.Method:A total of 82 patients with chronic renal failure uremia treated in our hospital from March 2016 to April 2017 were selected,according to the random number table method,they were divided intoobservation group(hemodialysis filtration) and control group(high throughput hemodialysis),41 cases in each group.The clinical effects,complications,serum creatinine level,immunoglobulin level and lung function index before and after treatment between two groups were compared.Result:The total effective rate of treatment in observation group was 97.56%,which was higher than 82.93% of control group( 字2=4.986,P=0.025).The incidence of complications in observation group was 14.63%,which was lower than 41.46% of control group( 字2=7.312,P=0.007).After treatment,the levels of serum creatinine and RV in two groups were all lower than those of before treatment,the levels of IgM,IgA,IgG,F(xiàn)VC,VC and TLC were all higher than those of before treatment,and the levels of serum creatinine and RV in observation group were lower than those of control group,the levels of IgM,IgA,IgG,F(xiàn)VC,VC and TLC were all higher than those of control group,the differences were statistically significant(P<0.05).Conclusion:High throughput hemodialysis and hemodialysis filtration in treatment of chronic renal failure uremia can effectively improve the immune function of patients and promote the effective improvement of pulmonary function,but compared with hemodialysis filtration,the effect of high throughput hemodialysis is more remarkable.
