IFN—γ含量測(cè)定在結(jié)核性胸膜炎患者臨床診斷中的效果分析

【摘要】 目的:探究IFN-γ含量測(cè)定在結(jié)核性胸膜炎患者臨床診斷中的應(yīng)用效果。方法:選取本院2016年1月-2017年12月呼吸內(nèi)科收治的50例結(jié)核性胸膜炎患者為研究組及非結(jié)核性胸膜炎50例患者為對(duì)照組,同期選擇院內(nèi)體檢的健康者50例為健康組,均采用酶聯(lián)免疫吸附試驗(yàn)對(duì)研究組及對(duì)照組患者進(jìn)行胸腔積液,并對(duì)3組患者外周血血清中IFN-γ含量進(jìn)行測(cè)定并比較。結(jié)果:經(jīng)測(cè)定后,研究組患者胸腔積液中IFN-γ濃度為(1 460.65±100.76)pg/mL顯著高于對(duì)照組(250.89±30.53)pg/mL,差異有統(tǒng)計(jì)學(xué)意義(P<0.05),研究組患者血清中IFN-γ濃度為(75.32±13.81)pg/mL顯著高于對(duì)照組(42.45±10.33)pg/mL及健康組(43.67±12.08)pg/mL,差異有統(tǒng)計(jì)學(xué)意義(P<0.05);研究組患者ROC曲線分析結(jié)果顯示,胸腔積液中IFN-γ濃度300 pg/mL為臨界值,IFN-γ濃度的測(cè)定對(duì)結(jié)核性胸腔積液診斷的特異度為93%,診斷靈敏度為98%。50例結(jié)核性胸膜炎患者治療后的總有效率為90.0%,2例出現(xiàn)胸膜肥厚粘連,其余患者均無嚴(yán)重并發(fā)癥及不良反應(yīng)發(fā)生。結(jié)論:對(duì)胸膜炎患者進(jìn)行酶聯(lián)免疫吸附試驗(yàn)檢測(cè)胸腔積液及外周血血清中IFN-γ含量可顯著提升結(jié)核性胸膜炎的診斷效能,值得臨床推廣采用。

【Abstract】 Objective:To explore the clinical application of IFN-γ content determination in the clinical diagnosis of tuberculous pleurisy.Method:50 cases of tuberculous pleurisy patients in our hospital from January 2016 December to 2017 Department of respiratory medicine were selected as research group and 50 patients of non tuberculous pleurisy in the control group,selected the same period in the hospital physical examination of 50 healthy cases as healthy group,they were given enzyme-linked immunosorbent assay to study group and control group of patients with chest effusion, and 3 groups of patients with IFN-γ content of peripheral blood serum were measured and compared.Result:After the test,the study group with pleural effusion in IFN-γ concentration was (1 460.65±100.76)pg/mL significantly higher than the control group (250.89±30.53)pg/mL,the difference was statistically significant (P<0.05).The study group in the serum of patients with IFN-γ concentration was (75.32±13.81)pg/mL significantly higher than the control group (42.45±10.33)pg/mL and healthy group (43.67±12.08)pg/mL,the differences were statistically significant(P<0.05).Patients in the study group ROC curve analysis showed that the concentration of IFN-γ in pleural effusion was 300 pg/mL as the critical value,the determination of IFN-γ concentration on the diagnosis of tuberculous pleural effusion.The specificity was 93%,sensitivity was 98%.In 50 cases of tuberculous pleurisy,the total effective rate was 90%,2 cases of pleural thickening and adhesion,and the remaining patients had no serious complications and adverse reactions.Conclusion:Enzyme linked immunosorbent assay for pleural effusion and peripheral blood serum IFN-γ content in patients with pleurisy can significantly improve the diagnostic efficacy of tuberculous pleurisy,which is worthy of clinical promotion.
